Before your appointment…
INSURANCE
Please call us and your insurance to verify coverage with us.
We accept most insurances, generally no ACO or MCO plans.
To avoid billing issues, please call your insurance and update your PCP to Dr. Justin Holtzman (NPI 1669769501). Note the reference number and bring it to your first appointment.
Please have your old PCP’s office fax your medical records to us. They can fax it to us at (781) 682-6157.
If they require paperwork, please complete a Release of Information form.
Make sure to check of “Complete Record” in section 4 and indicate the date range from your birthday to today. After you have completed the form and signed at the bottom, you can upload it to your patient portal or email it to us at admin@holtzmanmedical.org.
